Here is how to change your Medicaid Plan.

First, if you haven't already done so, we strongly recommend you create an account on the Florida Medicaid Member Portal as an alternative to calling Florida Medicaid and waiting on the phone. The Florida Medicaid Member Portal Medicaid allows you to request a different Medicaid Plan. Please select Community Care Plan. If given a choice for Provider, select Diana McLaughlin, MD, NPI 1528132909. Please record the confirmation and text it to us with the plan effective date.

If you do not have a web portal account and do not want to create one, please call Florida Medicaid Helpline at 1-877-711-3662. Please select Community Care Plan. You may change your Medicaid Plan during one of three events

  • Medicaid Open Enrollment which runs from October 1 to November 30 each year.
  • "For Cause". This happens when your PCP is no longer with the plan or if you moved.
  • Within 60 days of obtaining or renewing your Medicaid.
